Qu’est ce que c’est le syndrome du choc toxique ?

<p>Le choc toxique est une infection <strong>très rare mais très grave</strong> qui est provoquée par la prolifération du staphylocoque doré. Ce staphylocoque n’est pas forcément mauvais : il est présent naturellement dans notre organisme et dans la <a href="https://lapause.jho.fr/article/tout-savoir-sur-le-vagin-et-la-vulve/">muqueuse vaginale</a>. Mais certaines femmes sont porteuses d’une souche particulière de cette bactérie <em>staphylococcus aureus</em> qui peut devenir dangereuse si elle se développe.</p>
<h2>Les causes : une souche de staphylocoque spécifique</h2>
<p>Pour avoir des risques de déclencher un SCT, plusieurs facteurs doivent être réunis : </p>
<ul>
<li>La souche de staphylocoques dorés présents dans la flore vaginale de la personne menstruée doit être producteur d'une toxine particulière appelée TSST-1 (1-4% des femmes seraient porteuses de la <em><strong>bactérie S. aureus</strong></em>)</li>
<li>La personne menstruée doit <a href="https://lapause.jho.fr/article/que-se-passe-t-il-dans-notre-corps-pendant-les-regles/">avoir ses règles </a></li>
<li>Elle doit avoir gardé trop longtemps une protection hygiénique intravaginale, aussi appelées protections périodiques internes (cup, tampon, éponge). Les protections externes (<a href="https://lapause.jho.fr/article/comment-utiliser-une-serviette-hygienique/">serviettes</a>, <a href="https://lapause.jho.fr/article/comment-utiliser-une-culotte-de-regles/">culottes menstruelles</a>) présentent aussi des risques si elles sont utilisées dans de mauvaises conditions sanitaires.</li>
</ul>
<p>La stagnation du sang dans le vagin (à cause du port trop prolongé d’une cup ou d’un tampon) peut provoquer un milieu de culture favorable à ce staphylocoque, qui va se multiplier et libérer la toxine TSST-1 qui va alors passer dans le sang.</p>
<h2>Les symptômes du syndrome</h2>
<p>Les premiers symptômes sont la fièvre, l'hypotension (soit une baisse de la pression artérielle), les maux de tête pouvant aller jusqu'aux vomissements, les douleurs musculaires, des troubles intestinaux (diarrhées) ou encore des éruptions cutanées qui ressemblent à un coup de soleil. Les symptômes du début de cette infection bactérienne ressemblent donc fortement à ceux de la grippe ou de la gastro-entérite. La personne souffrant de cette maladie rare peut faire un malaise et perdre connaissance. </p>
<p>En effet, une fois passée dans la circulation sanguine, la toxine va attaquer les organes vitaux qui vont se mettre en mode « survie » et cessent petit à petit d’irriguer tous les membres, ce qui, dans les cas les plus graves, peut provoquer des nécroses ou même de manière extrêmement rare, la mort de la patiente. </p>
<h2>Traitement de la maladie</h2>
<p><span>Si vous avez les symptômes du CTS, <a href="https://lapause.jho.fr/article/comment-utiliser-une-cup-menstruelle/">retirez immédiatement la coupe menstruelle </a>ou le tampon et rendez-vous aux urgences de l'hôpital. Souvent, le traitement passe par une intraveineuse puis la prise d'antibiotiques. Généralement la patiente est admise dans les services de réanimation. Dans les cas très très rares, la patiente devra subir une intervention chirurgicale d'amputation s'il y a gangrène. </span></p>
<h2><strong>Une bonne hygiène menstruelle en prévention</strong></h2>
<p>Pour réduire le risque de contracter un CTS, voici les <a href="https://lapause.jho.fr/article/l-hygiene-pendant-les-regles/">mesures hygiéniques menstruelles à respecter </a>: </p>
<ul>
<li>Avant de mettre ou de retirer vos protections périodiques, <strong>il est important de bien se laver les mains</strong></li>
<li><strong>Bien stériliser vos coupes menstruelles </strong>si vous êtes utilisatrices de cup</li>
<li> Il est recommandé de <strong>changer votre tampon (et toutes vos protections hygiéniques) toutes les 4 a 6 heures. </strong>Pas de tampon ni de coupe menstruelle la nuit donc : privilégiez la culotte de règles ou les serviettes hygiéniques. Il ne faut pas garder trop longtemps sa protection.</li>
<li data-insertor-excluded="1"><strong data-insertor-excluded="1">Bien choisir son tampon en fonction de son flux menstruel : </strong>les tampons pour flux abondant lorsque vous avez un flux normal ou faible vont favoriser la stagnation du sang des menstruations car ils sont trop absorbants. Souvent, les jeunes filles sont concernées par cette maladie potentiellement mortelle en raison d'un port prolongé de tampons pour éviter les fuites.</li>
<li data-insertor-excluded="1"><strong>Si vous avez déjà eu un choc toxique staphylococcique menstruel, ne portez pas de protections périodiques intravaginales </strong>(pas de tampon, ni de cup, ni d'éponge) pour éviter la récidive.</li>
<li data-insertor-excluded="1"><strong>Ne pas porter de tampon si vous n'avez pas vos règles.</strong></li>
</ul>
<p>Les personnes menstruées victimes d'une <a href="https://lapause.jho.fr/article/precarite-menstruelle-quand-les-protections-hygieniques-sont-un-luxe/">situation de précarité menstruelle</a> sont plus à risque de contracter un syndrome du choc toxique.</p>
<h2><strong>La composition des tampons est-elle responsable du SCT ?</strong></h2>
<p>L'ANSES a enquêté sur la question du lien entre la composition des tampons périodiques en cellulose (et des protections hygiéniques conventionnelles) et le Syndrome du Choc Toxique en 2018. Les conclusions de cette étude soulèvent la présence de substances toxiques et de produits chimiques dans les tampons (perturbateurs endocriniens, herbicide, glyphosate, phtalates...) mais l'Agence n'établit pas de lien direct entre ces traces et la maladie. La mauvaise utilisation des tampons ou des mesures d'hygiène menstruelle inadaptées seraient donc à l'origine du déclenchement du SCT. </p>
<p>Le <a href="https://www.chu-lyon.fr/syndrome-du-choc-toxique-lie-aux-regles">Centre National de Référence des Staphylocoques</a> (CNR) a mené deux enquêtes qui débouchent aussi sur les mêmes conclusions : le port prolongé des tampons le jour ou la nuit favoriserait le déclenchement de cette maladie infectieuse. Ces enquêtes ont été lancées en raison d'une recrudescence inexpliquée des cas de SCT en France depuis les années 90. <a href="https://www.lemonde.fr/sante/article/2020/01/21/tampons-cups-les-experts-demandent-aux-fabricants-une-information-plus-claire-sur-le-choc-toxique_6026709_1651302.html">En moyenne, 20 cas de syndrome du choc toxique sont recensés par an en France.</a></p>
